A teaching hospital in Seattle was recently cited for negligence in maintaining sterile drug mixing and compounding areas for a period of 17 months, including in appropriate handling and identification of expired medications. Improper maintenance of compounding rooms and expired medication puts patients at risk for either ineffective treatment or an adverse reaction to expired drugs that may be subject to changes in their chemical and physical properties.
